A Gaming Vendor is any business or entity that distributes, manufactures, leases or sells Gambling equipment and/or video games of chance to the Prairie Island Indian Community, or any of their Gaming Enterprises.

New Vendor License
All Initial (new) Gaming Vendors Applicants for Class D Gaming Vendor License shall be required to submit the following forms to the Prairie Island Gaming Commission Vendor Licensing Department.
On-line Initial or New Gaming Vendor Application Form (These
forms can be FAXED or MAILED BY U.S. MAIL OR E-MAIL OR DELIVERY SERVICE).
Personal History Statements on any individual holding 5%
or more interest on the business entity, and any individual in direct
contact with Treasure Island Resort and Casino, Bingo/Pull Tab. (At least
2-3 persons shall complete these statements.) This includes a Photo
ID.
"Finger Print Cards" can be requested from the
Vendor Licensing Department. These shall be mailed to the Gaming
Vendor Applicants.
A SIGNED AND NOTARIZED "Authority to Release Information Form" for Class D Gaming Vendor License Applicants shall be sent with each person's "Personal History Statement".
A SIGNED AND NOTARIZED "Authority to Release All Claims Form" for Class D Gaming Vendor License Applicants shall be sent with each person's "Personal History Statement".
A Copy of ANY AND ALL CURRENT State Licenses.
A List of All Jurisdictions and Gaming-Related Entities the Gaming Vendor Applicant is licensed in.
Any Other Business Information necessary that pertains to the Gaming Industry.
A License Fee of $100.00 is required. Please Make Check Payable to: Prairie Island Indian Community
Renewal License
Gaming Vendor Class D License Renewal Applications are to
complete the On-line questionaire (This can be FAXED or MAILED BY U.S. MAIL
OR E-MAIL OR DELIVERY SERVICE).
Personal History Statements on any individual holding 5%
or more interest on the business entity, and any individual in direct
contact with Treasure Island Resort and Casino, Bingo/Pull Tab. (At least
1-2 who were not a part of the prior license year's intital (new) or renewal
Gaming Vendor Application.) A Photo ID shall be included.

All vendors must abide by all rules and
regulations set forth in the Prairie Island Gaming Ordinance and or Tribal/State
Compact, and any other rules or regulations set forth by the National Indian
Gaming Commission.
